{"id":1132,"date":"2025-10-02T06:46:36","date_gmt":"2025-10-02T06:46:36","guid":{"rendered":"https:\/\/dev.qrolic.com\/spatheory\/?page_id=1132"},"modified":"2025-11-05T06:15:01","modified_gmt":"2025-11-05T06:15:01","slug":"consent-forms","status":"publish","type":"page","link":"https:\/\/dev.qrolic.com\/spatheory\/consent-forms\/","title":{"rendered":"General Liability Release Form"},"content":{"rendered":"\n<p>By clicking the button \u201cConsent to Spa Services\u201d you agree to the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>I give my permission to receive a spa service from Spa Theory and the provider performing the service.<\/li>\n\n\n\n<li>I release Spa Theory and the provider performing my service of any liability associated with any injuries and \/or current and future conditions resulting from the procedures or products.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">General Liability Release Form: Massage Consent<\/h3>\n\n\n\n<p>By clicking the button \u201cConsent to Spa Services\u201d you agree to the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>I give my permission to receive massage therapy.<\/li>\n\n\n\n<li>I understand that therapeutic massage is not a substitute for traditional medical treatments or medications.<\/li>\n\n\n\n<li>I understand that the massage therapist does not diagnose illnesses or injuries, or prescribe medications.<\/li>\n\n\n\n<li>I have clearance from my physician to receive massage therapy.<\/li>\n\n\n\n<li>I understand the risks associated with massage therapy, including but not limited to: superficial bruising, short-term muscle soreness, exacerbation of undiscovered injuries. I therefore release Spa Theory and the massage therapist performing the massage from all liability concerning these injuries that may occur during the massage session.<\/li>\n\n\n\n<li>I understand the importance of informing my massage therapist of all medical conditions and medications I am taking, and to let the massage therapist know about any changes to these. I understand there may be additional associated risks to these.<\/li>\n\n\n\n<li>I understand it is my responsibility to inform my massage therapist of any discomfort I may be feeling during the massage session so they may adjust accordingly.<\/li>\n\n\n\n<li>I understand that I, or the massage therapist, may terminate the massage session at any time.<\/li>\n\n\n\n<li>I release Spa Theory and the massage therapist performing my service of any liability associated with any injuries and \/or current and future conditions resulting from the procedures or products.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">General Liability Release Form: Facial Consent<\/h3>\n\n\n\n<p>By clicking the button \u201cConsent to Spa Services\u201d you agree to the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>I acknowledge that my skin might experience temporary irritation, tightness, redness or slight swelling which usually dissipates within 72 hours depending on skin sensitivity.&nbsp;<\/li>\n\n\n\n<li>I acknowledge that if I am allergic to one or more ingredients in the products used, I may experience allergic reactions.<\/li>\n\n\n\n<li>I acknowledge that if I fail to use a minimal sunscreen (SPF45), I am more susceptible&nbsp;to sunburn, skin damage &amp; hyperpigmentation.
